On Sale and/or Sunday Certification completed and signed by city clerk
upon approval of On Sale or Sunday liquor license within ten days.
Liquor liability insurance covering the entire license period in the exact
corporate name of licensee.
Workers Compensation Insurance.
$20 buyers card application.
Current Minnesota Sales Tax I.D. Number. 
The state of Minnesota does not approve city-issued on sale licenses, 
but does require this certification within 10 days. This requirement 
must be met before liquor licensees are given approval to receive 
liquor shipments from Minnesota wholesalers.
